Our dedicated team works together to provide quality of life and comfort for each patient during end of life living.
CWLJ Hospice has a team of caring Volunteers that are available to come to your home. They assist with visiting,reading,walks, bringing music into the home and are able to assist with light household maintenance. They are able to provide respite care for main caregivers so they can run errands .
Offering Grief support for patients families as well as the general community. Support Groups and Individual Grief sessions are available.
Spiritual Services
Hospice Chaplain provides spiritual support to our patients and their families.Making spiritual assessments and assisting medical staff in developing individualized care plan including daily spiritual consultations and performance of sacraments as desired by patient.
Equipment Services
Grief Support
CWLJ Hospice provides each Patient with the necessary medical equipment/Supplies needed to live independently as possibly. Including medical beds, wheelchairs, oxygen machines, shower benches and more.
Social Services
Services include advising family, providing patient education and counciling and making referrals for other needed services.
